Anyone who cares about someone's wellbeing

You don't need to be a professional, and you don't need all the answers yet. A referral can come from anyone close to the situation.

Parents & Family

You know your child or loved one best. You can refer directly, any time.

Social Workers

We work closely with local authority and social care teams on referrals and reviews.

Clinicians & Discharge Teams

Including hospital discharge coordinators, nurses and GPs referring for complex needs.

Self-Referral

Adults can refer themselves directly, no professional referral needed.

How we handle what you tell us

Safeguarding sits behind everything we do. Any information shared with us, whether in a referral form, a phone call, or an initial conversation, is handled confidentially and in line with UK data protection law, and is only ever used to help us understand and meet the needs of the person being referred.

If a referral raises an immediate safeguarding concern, our team is trained to recognise it and will always act appropriately and quickly, working alongside the relevant local authority or safeguarding board. You will never be judged for asking a difficult question or raising a concern. That's exactly what we're here for.

Referral FAQs

Do I need a diagnosis or care plan already in place?

No, share whatever information you have. If a diagnosis or formal care plan isn't in place yet, we can still start the conversation and work out next steps together.

I'm not sure which service is right, can I still refer?

Absolutely. Select "Not sure" on the form, or just give us a call. We'll help you work out the right fit during the initial conversation.

Can I refer on someone else's behalf?

Yes, family members, social workers and healthcare professionals can all refer on behalf of someone else. We'll always involve the person being referred (and their family) as early as possible.
